Title:                       W-4 Form  

Learner Level:       Level 2 – 3 (could be higher if reading only)

Pre-Instruction:      Instruct students to bring in a list of all their income (first job, second job, interest income…)

Duration:                45 minutes - 1 hour    

Materials:               access to computer lab, tax information from home

Objectives:             The learner will become familiar with the W-4 form and related vocabulary. The learner will score at least 80% on the assessments.

Procedure:    1. The students type in the URL. (www.cdlponline.org)

                      2. Click on “working”     

                      3. Scroll down to “W-4 Form”

                      4.  Read/Listen to the story. Click NEXT

                      5.  Complete “Learn New Words”

                      6.  Complete “Spell a Word”

                      7.  Complete “Pick an Answer”

                      8.  Complete “What Did You Learn?”

                      9.  Complete “Write and Answer”

                      10. Click on the web sites listed and fill in the forms.

                      11. Click on “Activities/Scores”

                      12. Click “email them to someone”

                      13. Fill in the form; submit by clicking “Email My Scores”

                      14. Clear the Form

Evaluation:  Upon receiving their emails, I will be able to evaluate if the students reached their goal of at least 80% correct.
